Making a claim

If your claim relates to treatment in hospital, your claim will be automatically completed for your signature and forwarded to CUA Health for settlement. You need only pay any out of pocket costs that may be applicable to the cover you have selected.

When using your cover under extras benefits, you may use your CUA Health membership card for on-the-spot claims at participating HICAPS (health insurance claims and payments service) or iSoft (IBA Health Group Limited) registered provider. 

Alternatively you can simply complete a PDF icon claim form, attach both the bill and receipt, then either post it to CUA Health, or forward through your local branch.
