Your personal information
Information such as your State of residence, age and income thresholds affect the premium and the application of the Australian Government Rebate and Lifetime Health Cover loading. You agree that the information you have provided about yourself and your family members covered under the policy are correct.
From time to time, we may need to make changes to your cover. Before we do, we’ll generally write to you so it’s also important that you keep your contact details updated with us. Unless you request otherwise, we’ll contact you via the email address you nominate when you first join CUA Health.
The benefits we pay under your policy is determined under our Fund Rules, which are summarised in our Product Summary for the cover you’ve selected together with our Member Guide. You acknowledge that you are aware of and understand the relevant conditions (including any restricted or excluded services, and limits) which apply to your chosen cover.
If you have an Extras policy, switching health insurers or your cover will not re-set your annual limits for the calendar year.
If you’re new to private health insurance or you’re upgrading your cover (including reducing any Excess payable under your policy) then waiting periods may apply. Different waiting periods apply for different services. Refer to our Product Summary and Member Guide for full details.
To qualify for the 4% discount, you must pay your premiums by direct debit from a Great Southern Bank transaction account. We may amend or remove the discount and any qualifying criteria at any time, but we will always provide you with reasonable notice before doing so.
We collect information about you and your family members covered under your policy so we can set up and administer your private health insurance policy. Without this information we can't provide you with our products.
